Visual impairment linked to mortality in diabetes

Investigators were aware that visual acuity loss is a common consequence of type 2 diabetes and may complicate diabetes self-management. They investigated patients with newly-diagnosed type 2 diabetes to assess whether loss of visual acuity was an independent risk factor for mortality in this population. They found that impaired visual acuity at diagnosis was associated with increased all-cause mortality, independent of other risk factors for mortality.

Blood pressure variability linked to CV outcomes in diabetes

Blood pressure variability (BPV) is considered a novel risk factor for cardiovascular disease, as clinical trials have confirmed that short- and long-term variations in blood pressure independently contribute to target organ damage, cardiovascular events and mortality. In this systematic review and meta-analysis, investigators examined the relationship between BPV and all-cause mortality, major adverse cardiovascular events, microvascular complications and organ damage in type 2 diabetes. They found that in adults with type 2 diabetes, systolic BPV was associated with adverse outcomes and concluded that variation in blood pressure provided information beyond mean blood pressure on the risk of cardiovascular disease in these individuals.

Controversial metformin recommendations in new ESC guidance

The latest European Society of Cardiology guidelines on diabetes, pre-diabetes and cardiovascular diseases recognise that considerable emerging evidence should have implications for daily clinical practice. Controversially, metformin is no longer recommended as first-line treatment in type 2 diabetes. Suggestions for the prevention of cardiovascular disease, advice on lifestyle intervention, new lipid and blood pressure targets, a novel approach to glucose-lowering treatment, diabetes treatment aimed at reducing heart failure risk, and cardiovascular risk management are included. The guidance has been developed in collaboration with the European Association for the Study of Diabetes and updates recommendations made in 2013.

CREDENCE trial shows renoprotective effect of canagliflozin

Results of the CREDENCE (Canagliflozin and Renal Events in Diabetes with Established Nephropathy Clinical Evaluation) trial have been published in the New England Journal of Medicine, demonstrating a 30% reduction in renal outcomes with canagliflozin compared with placebo in people with type 2 diabetes and chronic kidney disease (CKD).

Bariatric surgery reduces microvascular outcomes in diabetes

Bariatric surgery improves glycaemic control and may modify cardiovascular risk in type 2 diabetes (T2D), but less is known about microvascular outcomes. Researchers performed a cohort study to examine the relationship between bariatric surgery and microvascular complications from T2D. All participants had T2D, and those who had bariatric surgery were matched with a control group who had not had the procedure. These cohorts were followed for up to 7 years, at the end which the incidence of microvascular disease was lower in the bariatric surgery group.

Metformin treatment in people with T2D and chronic kidney disease

Metformin is the first-line drug of choice in the treatment of type 2 diabetes in most diabetes guidance. Last year the European Medicines Agency (EMA) altered the restrictions on metformin use in people with diabetes and researchers wanted to discover, in the absence of prospective safety and efficacy studies, what was a safe, effective dose regimen for metformin in moderate and severe chronic kidney disease (CKD). Investigators concluded that metformin appeared to be safe, provided that there was dose adjustment for renal function. It was also found to be efficacious in moderate-to-severe CKD.
