Practitioner empathy impacts long-term diabetes outcomes

Patients and doctors value empathy. Empathetic care encompasses shared decision making and understanding the patient’s perspective as well as an appreciation of the broader context in which the patient’s illness is experienced. Patients from 49 English general practices were invited to assess practitioner empathy and their experiences of diabetes care in the 12 months since their diagnosis. Higher empathy scores were associated with lower risks of cardiovascular disease events and all-cause mortality.

Intensive intervention reduces stroke risk in diabetes with microalbuminuria

Data from the Steno-2 Study were analysed to assess whether multiple risk factor intervention reduced the chance of stroke in individuals with type 2 diabetes and microalbuminuria. Participants were randomly allocated to intensified or conventional multifactorial intervention targeting classical cardiovascular disease risk factors. Post hoc analysis showed that the risk of stroke and number of recurrent cerebrovascular events were reduced with intensified multiple risk factor intervention in patients with type 2 diabetes and microalbuminuria.

Preventing misdiagnosis of diabetes in the elderly

Investigators examining increased HbA_1c levels in older people without diabetes have found a positive association between HbA_1c concentration and age in two independent population-based cohorts. This increase was observed equally in lean, overweight and obese individuals. Investigators used their findings to propose age-dependent reference values for HbA_1c and suggested that clinicians be made aware of this age-related increase in HbA_1c that is independent of diabetes. They propose that age-dependent reference intervals be used to improve patient care and the diagnosis of diabetes.

Questions over metformin use in obese pregnant women

Australian investigators wanted to examine appropriate antenatal dietary and lifestyle advice for women who were obese or overweight during pregnancy in order to limit gestational weight gain and improve maternal and infant health. They performed a Cochrane database search for studies using metformin as an adjuvant therapy to current antenatal strategies in pregnant women with obesity. They found insufficient evidence to support metformin use in obese women during pregnancy to improve maternal and infant outcomes. However, there was an increased risk of adverse effects, particularly diarrhoea, in pregnant women taking metformin.
