Peak CVD and mortality risk in month after severe hypoglycaemia

People with diabetes who experience severe hypoglycaemia are at a high risk of cardiovascular events. Investigators wanted to clarify the temporal relationship between hypoglycaemia and cardiovascular events, so they conducted a large matched-control, longitudinal study. Within the first month following a severe hypoglycaemic event, there was a significant increase in the incidence of cardiovascular disease and all-cause mortality. These increased risks dropped significantly each month for 6 months after the severe hypoglycaemic event.

New consensus on hyperglycaemia management in type 2 diabetes

The American Diabetes Association and the European Association for the Study of Diabetes have published a new consensus guidance document on the management of type 2 diabetes in adults. The guidance does not take a target-driven approach, rather suggesting that patient factors and preferences should inform the process of individualising treatment goals and strategies. It points out that not all people living with diabetes should have the same level of diabetes control and that patient-centred care is vital. It also acknowledges that the goals of treatment for type 2 diabetes are to prevent or delay complications and maintain quality of life.

Association between severe hypoglycaemia and CV outcomes and mortality

In an analysis conducted as part of the study DEVOTE (Trial Comparing Cardiovascular Safety of Insulin Degludec vs Insulin Glargine in Patients with Type 2 Diabetes at High Risk of Cardiovascular Events), the incidence and rates of severe hypoglycaemia and all-cause mortality were analysed. There was a significantly higher risk of all-cause mortality for people who had experienced severe hypoglycaemia during the trial. There was also a higher risk of all-cause mortality up to 1 year after experiencing severe hypoglycaemia compared with no severe hypoglycaemia in the same time interval.
