ADA supports individualised nutritional therapy in prediabetes

Adults with prediabetes should be provided with individualised nutritional therapy and the latest American Diabetes Association (ADA) consensus report includes guidance on how to support these individuals. This consensus updates the 2014 ADA position statement and aims to provide clarity on the many food choices and eating patterns that can help people with prediabetes, type 1 or type 2 diabetes to achieve health goals and maintain quality of life. The guidance suggests that all members of the healthcare team should support nutrition therapy as an important component of quality diabetes care. Many of the suggested diets are critically appraised. The guidance promotes and supports healthful eating patterns, emphasising a variety of nutrient-dense foods, along with appropriate portion sizes, to improve health and well being.

Position statement defines remission of diabetes

The Primary Care Diabetes Society (PCDS) and the Association of British Diabetologists (ABCD) have issued a joint statement recognising that it is possible to achieve remission in type 2 diabetes, although there currently is no single internationally-acceptable definition of this condition. The statement proposes a practical definition of type 2 diabetes remission involving weight loss, achieving fasting plasma glucose of <7 mmol/L or HbA_1c <48 mmol/mol (6.5%) and the cessation of glucose-lowering therapies. They believe 6 months to be a reasonable duration over which any given level of glycaemia should be maintained before remission can be diagnosed.

Weight loss results in long-term diabetes remission

Sustained weight loss results in the long-term remission of diabetes in a third of patients, according to researchers involved in the Diabetes Remission Clinical Trial (DiRECT). In this ongoing study in obese patients with type 2 diabetes, 36% of participants given a low-calorie formula diet for the first 3 months and then a structured diet using normal food to support weight maintenance were still in remission at 2 years, compared to just 3% of the control group, who received best-practice care in accordance with guidelines. The original study demonstrates the importance of marked weight loss (>15 kg), which achieved a 46% remission of diabetes. This follow-up study shows a clear dose–response relationship between the amount of weight loss and remission of diabetes. About one in three obese people with type 2 diabetes, given the weight loss and maintenance advice, were able to control their blood sugar levels, reducing the need for medication.

High intake of antioxidant-rich foods can prevent type 2 diabetes

Investigators developed a unique scoring mechanismto predict the likehood of individuals developing type 2 diabetes (T2D). By utilising this tool, they identified a cohort of 402 female patients at very high risk of developing T2D, documented their risk factors, and specifically their dietary intake. This cohort was then followed up for 20 years. Investigators found that the top characteristic associated with not developing T2D, despite being at very high risk, was a healthy diet, characterised by limiting Western dietary habits and having a high intake of antioxidant‐rich foods.

Plant or animal food replacements in lower carbohydrate diet influence mortality risk

Further confirmation that low carbohydrate diets favouring plant-derived proteins from foods such as vegetables, nuts and peanut butter are associated with reduced mortality comes from a National Institutes of Health prospective cohort study and meta-analysis published in the Lancet Public Health journal.
