With evidence suggesting that over half of the UK population aged over 45 now has more than one long-term condition (LTC), the management of obesity, type 2 diabetes and the LTCs associated with them is growing ever more complex.

Despite the many challenges it faces, primary care is most strongly placed to provide effective, holistic care for these patients, in terms of both expertise and the relationship with the individual. This one-day conference from the Primary Care Diabetes & Obesity Society brings insights from the whole primary care multidisciplinary team to help healthcare professionals provide optimal care of multiple LTCs with the teams and the budgets they currently have.

Illustrated by cases at varying stages of their diabetes journey, the speakers will summarise the best-practice management of a host of conditions and complications, including obesity, type 2 diabetes, severe mental illness, cardiovascular and renal disease, microvascular complications, liver disease and frailty.

Attendees will learn not only what the latest guidelines recommend but also the principles that will allow them to provide the best possible care independent of new drug developments and the changing landscape of NHS services.
