New NICE guideline on type 1 diabetes in adults

Type 1 diabetes currently affects 370,000 people in the UK and should be treated by insulin replacement along with active management of cardiovascular risk factors. This latest guidance provides evidence-based, practical advice on supporting adults with type 1 diabetes to live unrestricted lives and reduce the risk of complications, but it recognises that, even with improvements in care, adults with type 1 diabetes, on average, have a reduced life expectancy, owing to the effects of persistent hyperglycaemia above accepted target levels.

This guidance updates the elements of CG15 relating to adults and recognises the challenges that remain in care provision for adults with type 1 diabetes, especially HbA1c above target levels, unacceptably high rates of diabetic ketoacidosis and an increased risk of renal failure. To help reduce the incidence of these, a target HbA1c of 48 mmol/mol (6.5%) is recommended, as is a minimal testing frequency of four times daily.

In managing the condition, multiple daily injections of basal–bolus insulin regimens are recommended, along with a detailed and agreed care plan that is individualised and culturally sensitive. It is also recommended that newly diagnosed people with type 1 diabetes be offered a structured education programme of proven benefit such as the DAFNE (Dose Adjustment for Normal Eating) programme. There is also detailed guidance on counting carbohydrates and following a low-glycaemic-index diet.
