Join us for a look at the intricate landscape of diabetes, obesity and other common long-term conditions, led by the Primary Care Diabetes Society in conjunction with experts from secondary care. A series of presentations will explore the latest guidelines, evidence, and practical strategies for navigating the intersection between diabetes and other long-term conditions – from cardiovascular disease to cancer. The intrinsic links between diabetes and obesity will also be explored, with a look at what weight management means in the context of multiple long-term conditions.

Gain valuable insights on how patient pathways between primary and secondary care could be optimised, and what practical steps can be taken to promote collaboration across the entire multidisciplinary team. The day concludes with a panel discussion on how to best prioritise care when faced with multiple long-term conditions needing urgent management.
